Intra-aortic balloon counterpulsation in acute myocardial infarction: too few or too many?
نویسنده
چکیده
The concept of counterpulsation was introduced over four decades ago by Clauss et al. (1) and adapted to intra-aortic balloon pumping (IABP) by Moulopaulus et al. (2) in 1962, and it remains the most common mode of cardiac assist. Intra-aortic balloon counterpulsation (IABCP) was first introduced into clinical practice by Kantrowitz et al. (3) in 1968 for the treatment of cardiogenic shock. Initially, the 12F to 14F intra-aortic balloon (IAB) catheter required surgical insertion via a dacron graft sutured to the femoral artery. In 1979, the IAB was adapted for percutaneous insertion by Bregman and Casarella (4), and this was followed in the early 1980s by a dual-lumen, wire-guided, smaller catheter device which could be placed in 5 min with significantly improved insertion success and reduced vascular complications (5–8). Refinements have reduced the catheter size to 8F and 9F for 30to 50-cc balloons, and technically advanced consoles can rapidly shuttle the helium gas with automatic timing.

Treatment Options in Cardiogenic Shock with Intra-aortic Balloon Counterpulsation
Cardiogenic shock (CS), a state of inadequate tissue perfusion due to cardiac dysfunction, remains the leading cause of death following acute myocardial infarction (AMI). While the prognosis of CS post-AMI has improved in recent decades due to advances in treatment modalities, the mortality rates remain unacceptably high (~40–50% according to recent registries and clinical trials). Current trea...

متن کاملSystematic review: Intra-aortic balloon counterpulsation pump therapy: a critical appraisal of the evidence for patients with acute myocardial infarction
Introduction Intra-aortic balloon counterpulsation pump (IABP) therapy has been used in several clinical situations, predominantly in critically ill patients, since 1968 [1]. In acute myocardial infarction (AMI) patients who are experiencing continued ischemia, IABP therapy may be used in an attempt to improve patency of an infarct-related coronary artery (IRA) and reduce the rates of recurrent...

OBJECTIVES We sought to examine contemporary utilization patterns and clinical outcomes in patients with acute myocardial infarction (AMI) requiring intra-aortic balloon pump (IABP) counterpulsation. BACKGROUND Despite increasing experience with and broadened indications for intra-aortic counterpulsation, the current indications, associated complications, and clinical outcomes of IABP use in ...
متن کاملProlonged circulatory support with the intra-aortic balloon pump after myocardial infarction.
Circulation was supported by intra-aortic balloon counterpulsation for 30 and 38 days respectively in two patients with cardiogenic shock after acute myocardial infarction. One was flown 1400 km to Cape Town for heart transplantation but died after being weaned from the pump while awaiting a suitable donor. The other underwent successful surgical closure of a ruptured ventricular septum on the ...
